WHEREAS, in order to protect abandoned dogs, FLOD VT does accept for placement from individuals, animal shelters, humane societies and other persons and entities, dogs which are homeless, and WHEREAS, FLOD VT does attempt to place such dogs where possible in suitable homes in consideration for the payment of a fee determined in each case. *

(By checking below) adopter acknowledges they have read, understand and agree to all of the following:

1. *

Adopter agrees to pay a fee of $500. The adoption fee for unaltered puppies is $400. In addition, the adoption fee may be reduced for senior or special needs dogs. The dog will have received age-appropriate vaccinations, deworming medications, and flea/tick/heartworm prevention. Other health-related services may have been administered as needed on the advice of a veterinarian.

It is expressly understood that FLOD VT places the dog with no warranty as to the physical condition or temperament of the dog. ADOPTER ACCEPTS THIS DOG AS IS WITH ALL DEFECTS, EITHER OBSERVABLE OR UNOBSERVABLE, AND ASSUMES ALL VETERINARY AND TRAINING COSTS FOR SUCH DOG ON RECEIPT OF THE DOG. Veterinary costs may include, but are not limited to, heartworm treatment if the dog tests positive after arrival in Vermont and medications for the eradication of intestinal parasites. Adopter agrees to engage a professional trainer if the dog exhibits unwanted behaviors that the Adopter cannot manage without professional assistance.

3. *

Adopter agrees to give the dog a minimum of two weeks to adjust to its new home and family. If the Adopter determines they cannot keep the dog (except in the case of euthanasia prescribed by a veterinarian), it is in the best interest of the dog to notify FLOD VT . If FLOD VT can assist with rehoming the dog, Adopter will allow FLOD VT two weeks to find placement in a suitable foster or permanent home. During this two-week period, FLOD VT requests that the Adopter keep the dog in their home or pay boarding fees until the dog can be moved into a new placement. In the event of a return, refund of the adoption fee is at the discretion of the FLOD VT Board of Directors.

4. * Adopter agrees to care for the dog in a humane manner, including, but not limited to, the following: *

A. Adopter will provide water, quality, food, shelter, physical and mental exercise and veterinary care, including vaccinations and heartworm prevention for the duration of the dog's life, in accordance with current veterinary practice recommendations.

B. Adopter agrees that the dog will be kept as a home companion and will share Adopter's home, being given the same considerations as would a human family member.

C. Adopter agrees to keep a collar with the dog's current rabies tag and family phone number on the dog at all times.

D. Adopter agrees that under no circumstances is the dog to be routinely tethered outside as a means of confinement.

E. Adopter will not use the dog, nor will they allow the dog to be used, for any illegal purpose. Adopter agrees that the dog will not be attack- or protection-trained and will not be used for guard work in any commercial endeavor.

5. *

If adopting a puppy and the puppy has not been altered before delivery to Adopter, Adopter agrees to spay/neuter their puppy before the age of 1 year at their cost, and to provide FLOD VT a spay/neuter certificate (or other proof) that the dog has been spay/neutered. Failure to do so will result in the forfeiture of the dog.

6. *

Should Adopter breach any of the agreements contained in this Adoption Contract, FLOD VT shall have the right to rescind this Adoption Contract and Adopter must return dog to FLOD VT immediately. Should Adopter fail to return the dog, FLOD VT may seek an Order from a Court of competent jurisdiction to facilitate the return of the dog. Adopter understands and accepts financial responsibility for all attorney and court fees related to this action.

7. *

This contract is to be governed by the laws of the State of Vermont.
